I have been considering adopting a former foster as I recognize they still need love, and support to succeed in life. How would one find those who may still wish to be adopted?

Are there any programs in PA (or other states) that help Aged-Out Former Fosters connect with supportive resource adults, find jobs, and possibly get on a holiday and/or birthday registry? Have been trying to find such so I can help some in my area.
